internals: document revlog format

It seems like a good idea to document the revlog format.

There is a lot more that could be added to this documentation.
But you have to start somewhere.

  $ mkdir a

  $ hg rm a
  removing a/b (glob)
  $ hg ci -m m a

  $ mkdir a b
  $ echo a > a/b
  $ hg ci -Am m
  adding a/b

  $ hg rm a

Relative delete:

  $ hg ci -m m ../a

  $ cd ..
