mq: remove qrefresh slow path (issue2025)
The slow path/fast path distinction has existed since mq was added to Mercurial
in 2006. The slow path was used whenever the refreshed revision wasn't the tip
Basic install:
$ make # see install targets
$ make install # do a system-wide install
$ hg debuginstall # sanity-check setup
$ hg # see help
