convert: add mapname parameter to checkrevformat

Upcoming patches will add new map files so we change the calling sequence of
checkrevformat so that error messages will let the user know which file has the
wrong rev format.

  $ rm -rf a
  $ hg init a
  $ cd a
  $ echo a > a
  $ hg ci -Am0
  adding a
  $ hg tag t1 # 1
  $ hg tag --remove t1 # 2
