transplant: add the transplanted revset predicate

This adds support to identify a particular transplanted changeset or set
of changesets. The argument is optional. Examples:

hg log -r 'transplanted(1234 or 2345)'
hg log -r 'transplanted()'

Basic install:

 $ make            # see install targets
 $ make install    # do a system-wide install
 $ hg debuginstall # sanity-check setup
 $ hg              # see help
