help: add internals.extensions topic

I think it's better to include the API overview in core as we now have the
internals section in our help system.

Retrieved from the wiki, and formatted as reST. Several wiki links are
removed since they are invalid in the help. The sections about example
extension are removed at all as they seemed too verbose.

This tests if hgweb and hgwebdir still work if the REQUEST_URI variable is
no longer passed with the request. Instead, SCRIPT_NAME and PATH_INFO
should be used from d74fc8dec2b4 onward to route the request.
