revlogio: speed up parsing
- precalcuate ending offset
- pull some variables into local scope
- separate inline and out of line code paths
Basic install:
$ make # see install targets
$ make install # do a system-wide install
$ hg debuginstall # sanity-check setup
$ hg # see help
