util: make wrap() require a width argument

This keeps hgweb's help engine from poking at file descriptors that
don't exist.

  $ hg init a
  $ hg clone a b
  updating to branch default

  $ cd a
  $ echo '[paths]' >> .hg/hgrc
  $ echo 'dupe = ../b' >> .hg/hgrc

  $ cd ..
  $ hg -R a in dupe
