changegroup: fold progress meter into callbacks

progress meters now start at 1 rather than 0

  $ hg init

  $ hg add a
  $ hg commit -m test
  $ rm .hg/requires
  $ hg tip

  $ echo indoor-pool > .hg/requires
  $ hg tip
