nodemap: use data from the index in debugnodemap --dump-new

It is better to run the actual code when generating a new nodemap cache from
scratch. So we do.

  $ mkdir a
  $ echo a > a/b
  $ hg ci -Am m
  adding a/b

  $ hg rm a
  removing a/b
  $ hg ci -m m a

  $ mkdir a b

  adding a/b

Relative delete:

  $ hg ci -m m ../a
